
What to Bring to Your First Appointment.
Completing your forms in advance is just one way to make your first visit as smooth as possible. Along with your paperwork, there are a few items we’ll need at check-in to get everything set up before you meet your orthodontist.
- Completed new patient forms (download below)
- Valid photo ID (driver's license, passport, or school ID)
- Dental insurance card (front and back)
- List of any current medications or supplements
- Any recent dental X-rays if available (we may take new ones)
- Parent or legal guardian for patients under 18

Medical History
Covers your health background, current medications, and any conditions relevant to your orthodontic care. Completed at check-in or sent to you in advance by email.
Insurance & Benefits
Allows our team to verify your orthodontic coverage before your visit. We handle the insurance verification — you just provide your carrier and member ID.
Financial Agreement
Reviewed together with your Treatment Coordinator at your consultation. We walk you through every detail so there are no surprises before you commit to any plan.

Your orthodontist reviews your file, performs your complimentary 3D digital scan, and walks you through every treatment option with a personalized plan and clear cost breakdown.
